Make an appointment for the initial sleep consult for patients who need a Comprehensive Sleep Evaluation & RX for a home sleep test.

If you are a medical provider, order the home sleep test using our order form or upload your RX, Clinical Notes, and Patient Information.

Once the Sleep Test has been ordered a SLEEP RESULTS VISIT can be scheduled (2 -3 days out).

A Credit Card must be left on file for the patient to leave the office with a Home Sleep Test.

Select the kind of sleep test being setup and login to the portal to enter the patients information.

Explain to the patient how to use the home sleep test or have them watch the instructional videos.
